Steps to Get Started
As a complement to our imaging services, we offer our referring providers the opportunity to utilize our Pre-Authorization services in which University Radiology Client Services obtains the preauthorization on behalf of the referrer and schedules the patient at one of our convenient University Radiology centers.

Below are the steps required to register for our Pre-Authorization service.
Review the Pre-Authorization
information sheet that describes our service.
Complete the Referring Practice Information form.
Review, sign and date the Business Associate Agreement.
Complete the Service Agreement.
Fax the completed forms to (732) 385-3267.
University Radiology Client Services will contact you 24-48 business hours from receipt of your faxed documents to determine a go-live date. At that time, we will work with you to set-up a Pre-Authorization Request Test patient.
